Varicocele treatment by embolization : An innovative, non invasive therapy.

Article supervised by Dr. Antoine Hakime, last updated on 14/09/2024.

Traditionally, surgery was the go-to treatment option, but today, there’s a less invasive and innovative alternative: varicocele embolization.

In this article, we’ll delve into this groundbreaking procedure, discussing what it is, how it works, and its advantages over traditional surgery. So, grab a comfy seat, and let’s explore this exciting treatment!

Varicocele embolization : a minimally invasive procedure to preserve fertility

Varicocele embolization is a cutting-edge medical procedure used to treat those enlarged veins within the scrotum. These veins are similar to the ones you see in your legs (varicose veins), but in this case, they occur in the testicles. These veins can be a real pain, causing discomfort and sometimes even fertility issues.

So, how does this embolization thing work? Well, it involves a minimally invasive approach, unlike traditional surgical treatments. A radiologist, a specialized doctor, performs this procedure by using imaging guidance to access and block off those pesky veins.

The Procedure

Here’s a step-by-step breakdown of what happens during this embolization:

  1. Preparation: The patient typically lies on an examination table, and the area around the scrotum is cleaned and sterilized.
  2. Local Anesthesia: To ensure the patient’s comfort, a local anesthetic is administered to numb the scrotal area.
  3. Guided Access: A small incision, usually in the groin or neck, is made. Then, a catheter (a thin, flexible tube) is inserted into the vein under the guidance of imaging techniques such as ultrasound or fluoroscopy.
  4. Blocking the Vein: Special materials or coils are introduced through the catheter into those problematic veins, blocking them off. This prevents blood from flowing through those affected veins, rerouting it to healthier ones.
  5. Removal of Catheter: Once the veins are sealed off, the catheter is carefully removed.
  6. Recovery: The patient is observed for a brief period to ensure there are no complications, and they can usually return home the same day.

Varicocele Embolization : Benefits

  1. Minimally Invasive: Unlike surgery, which requires a larger incision and more recovery time, embolization is minimally invasive. This means smaller incisions, less pain, and a faster recovery.
  2. Outpatient Procedure: This embolization is typically performed on an outpatient basis, meaning you can go home on the same day. Surgery often requires a hospital stay.
  3. Reduced Risk of Complications: The minimally invasive nature of embolization results in fewer complications compared to surgery, such as infection, bleeding, or scarring.
  4. Faster Recovery: Patients generally recover more quickly after embolization, allowing them to return to their normal activities sooner.
  5. Improved Fertility: These pesky veins can negatively impact fertility. By treating them with embolization, the chances of improving sperm quality and fertility are increased.
  6. Effective Pain Relief: Embolization can provide effective relief from the pain and discomfort associated with these veins, improving the patient’s overall quality of life.
  7. High Success Rate: Studies have shown that embolization is highly effective in treating these veins, with success rates comparable to surgery.

Candidates for embolization typically include :

– Men with symptomatic veins (pain or discomfort)

– Men with fertility issues related to these veins

– Individuals who prefer a less invasive approach to treatment

Your doctor will perform a thorough evaluation and discuss your options with you to determine the best course of action.

What is a varicocele ?

A varicocele is an abnormal enlargement of the veins within the scrotum, similar to varicose veins that can occur in the legs. It primarily affects the pampiniform plexus, a network of veins in the spermatic cord, which drains blood from the testicles.

Varicoceles are common, especially in younger men, and are more often found on the left side of the scrotum due to anatomical differences in how blood drains from that side. They are usually harmless but can sometimes lead to symptoms like pain, testicular atrophy (shrinkage), or fertility problems, as varicoceles can affect sperm production and quality.

Treatment may not be necessary unless symptoms arise, but if pain, testicular damage, or fertility issues occur

Varicocele Symptoms

The swollen veins might not always cause disturbance, but when they do, you’ll know it! Here’s what you might experience :

  1. Dull, aching pain in the scrotum or groin area
  2. Heaviness or discomfort in the testicle(s)
  3. Visible or palpable enlarged veins in the scrotum, often described as a “bag of worms”
  4. Shrinking (atrophy) of the affected testicle
  5. Fertility problems, such as difficulty in conceiving
  6. Swelling in the scrotum
  7. Increased pain after physical activity or standing for long periods
  8. Relief from pain when lying down

Many varicoceles are asymptomatic and discovered during routine examinations. However, when symptoms are present, they can vary in intensity.
